Does your office accept walk-ins?
Yes! We accept walk-ins for acute issues and situations you feel are an urgent concern. These things are evaluated on a case by case basis. Please call the office before coming it to make sure we have time and a provider available.
What should I bring to my appointment?
Please show up to your appointment 5 minutes early to check in. Bring with you your insurance card and a form of ID.

What if I need to cancel my appointment?
If you are unable to keep your appointment, please contact the office at least 24 hours in advance. There may be cancellation fees for missed appointments.
How do I get my prescription refilled?
If you need a refill of a medication that was prescribed by a provider in our office, please contact your pharmacy.
What if I feel I need to see a specialist?
If you feel that you need to see a specialist and you would like your primary care provider to refer you, please contact our office to schedule an appointment to discuss that with them.
What if I need a form filled out or signed by my provider?
If you have paperwork of any kind that you need filled out, your provider will need to have a visit with you to complete and sign. Please contact our office to schedule an appointment.
When is payment expected from patient?
Payment is due at the time of service. If you have insurance we will need to collect the co-payment or the full amount depending on your deductible. Payment plans are available to be discussed prior to your appointment. Our office is also able to work off a sliding fee scale.
What Insurance companies are you contracted with?
We accept most insurance companies but please call to verify.
